Ontario Community Paramedicine
Ontario Community Paramedicine is a patient-centred model of care that extends the expertise of paramedics beyond emergency response to support proactive, community-based healthcare delivery. Working in collaboration with primary care providers, hospitals, home and community care organizations, and other health system partners, community paramedics provide in-home assessment, chronic disease monitoring, health promotion, post-discharge follow-up, and care coordination for individuals with complex health needs.
The model aims to improve access to care, enhance patient outcomes, support aging in place, and reduce avoidable emergency department visits, hospital admissions, and 911 utilization across Ontario.

Core Services

Clinical Practice Guidelines
Community Paramedicine programs across Ontario use evidence-informed Clinical Practice Guidelines to support consistent, safe, and high-quality patient care. These guidelines assist community paramedics in clinical assessment, treatment, monitoring, and referral processes while promoting integration with primary care, home and community care, specialist services, and Ontario Health Teams. By supporting standardized care delivery and collaborative decision-making, Clinical Practice Guidelines help improve patient outcomes and strengthen access to care within the community.
