How Referrals Work
Community Paramedicine services may be accessed through a variety of referral pathways, depending on local program availability and patient needs.
Referrals may come from:
- Family physicians and nurse practitioners
- Specialists and community health providers
- Ontario Health atHome care coordinators
- Long-term care and retirement home partners
- Hospitals and discharge planning teams
- Paramedics responding to 9-1-1 calls
- Emergency departments
- Community support service organizations

In some communities, patients, family members, or caregivers may also be able to contact their local Community Paramedicine program directly to inquire about services.
